Consent to the processing of personal data
I (an individual), being the subject of financial data, in accordance with the Russian Federation law on personal data, in particular Federal Law No. 152-FZ of July 27, 2006, "On Personal Data" (hereinafter referred to as the "Personal Data Law"), confirming my legal capacity, acting freely, of my own free will, and in my personal interests, provide recommendations on the handling of personal data (hereinafter referred to as "Consent") to JSC "DE-5" (TIN) 7706697690, OGRN 5087746109908) (hereinafter referred to as the "Operator") on the following terms:

Consent is given to the processing of the following personal data: last name, first name, patronymic, contact telephone numbers; Email address, user data (including location information); OS type and version; Browser type and version; device type and screen resolution; the source from which the user accessed the site; OS and browser language; which pages are opened and which buttons the user clicks; IP address (does not apply to determining the category of financial data or biometric personal data).
4. Consent is provided for the processing of my medical data for the following purposes:
5. I authorize the following actions to be performed on my personal data during processing: collection; recording; systematization; accumulation; storage; clarification (updating, modification); retrieval; use; transfer (provision, access); blocking; deletion; destruction.
6. I authorize the transfer of data to third parties for the purposes specified in the agreement, payment processors and banks, and for the distribution of services, if necessary for the purposes specified above, as set out in the Policy.
7. I consent to contact me consciously and voluntarily for the period until the termination of the activity; the Operator's removal has been unilaterally revoked at any time.
8. I am informed that this Consent may be revoked in accordance with Part 1 of Article 2 of Article 9 of the Law on Digital Data by sending requests for a signature review (written statements or documents signed in accordance with the legislation of the Russian Federation in the field of electronic receipt) to the following email address: info@dental-expo.com with the subject "Withdrawal of Agreement on Processing Medical Data."
9. In the event of withdrawal of consent to process medical data, the Billing Operator will continue processing medical data without my consent if there are grounds specified in paragraphs 2-11 of Part 1 of Article 6, Part 2 of Article 10, Part 10. 2 of Article 11 of Federal Law No. 152-FZ "On Financial Data" of July 27, 2006, "On Financial Data," and other personal data of a temporary permit or will be anonymized within 30 days of receipt of the written revocation of this Consent.
10. This Agreement shall apply at all times until the data processing changes specified in paragraphs 7 and 8 of the Consent.
11. The terms of this Agreement may be amended and/or supplemented by the Operator unilaterally without any special notice. The current version of the Agreement is an open and publicly available document.

- use of the Operator's Services and Resources, as described in the Personal Data Processing Policy (hereinafter referred to as the Policy);
- registration of a personal account;
- identification of me as the User for the provision and rendering of services;
- provision of feedback by the Operator, including sending requests, inquiries, current use of the Operator's Services and Resources, provision of services, processing requests and proposals from me as a User, consulting me on the services offered; fulfillment of obligations to me as a User under an agreement concluded with the Operator (in particular, a public offer for the provision of services and other contracts, Agreements executed by the Operator). Execution, in particular, includes informing about the date and time of events, classes, and webinars (including, but not limited to, sending SMS messages, phone calls, using instant messengers and social networks, sending notifications to the email address provided to me as the User);
- confirmation of the accuracy and completeness of the digital data provided by the User;
- posting on the Operator's Services and Resources, in official network groups and other social communities of the Operator on the Internet, with the exception of advertising and information sources, in cases not included in the list with the verification of my identity:
- video, photo, and text materials submitted during the provision of services;
- feedback left by me as the User on the services provided by the Operator.
- providing me, as the User, with effective customer and technical support in resolving issues related to the use of the Site and/or the provision of services;
- improving the quality of service for Users and the Operator's modern services and Resources by processing requests and applications from me as User;
- conducting metrics and other research based on the anonymized information provided to me as the User.
